ENROLLMENT APPLICATION

We consider enrolling children who are between the ages of 6 weeks to 4 years old.  To initiate your child’s enrollment into our center, please follow these instructions:

1. Open and print the enrollment application form using the Adobe Acrobat Reader
2. Answer the questions, sign and then date the form
3. Mail the form or schedule an appointment to drop-off the form
